The case managers mainly work in the community and manage patients that are housebound and suffer from multiple medical problems. They manage patients at home to optimise the treatment of their conditions and to prevent avoidable hospital admissions.

Clinical Pharmacists can consult with patients and prescribe medication independently. They also perform medication audits to ensure patients are prescribed medications safely and are monitored appropriately. Clinical Pharmacists can also issue repeat medications and send the scripts electronically.
Community midwives provide antenatal and postnatal care to our ladies. They have regular clinics at the health centre. Please ask at reception about the procedure to book to see a midwife if you are pregnant and how to inform us about a pregnancy.
A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
We have a counsellor who attends the practice weekly. Patients will need to be referred to the counsellor by a GP. Please book to see a GP to discuss any concerns.
